Building Request Form

NOTE: This form was updated 2/11/10.  Please read through all of this information before proceeding.  The purpose of this form is to submit requests to reserve portions of the church building for meetings or functions by members.  Please read all the following terms and conditions before submitting a request.

Rules for Building Usage

1. A building request form must be completed for each date of usage.  If you have a reoccuring event, please specify that in the notes below.  Building requests must be submitted and approved before any event is to take place.  PLEASE DO NOT SUBMIT A BUILDING REQUEST THE NIGHT BEFORE AN EVENT.  PLEASE BE RESPECTFUL AND COMPLETE A BUILDING REQUEST AT LEAST 2 DAYS PRIOR TO YOUR EVENT. 

2. The person responsible for locking/alarming the building and the person responsible for clean up MUST BE SPECIFIED on the building request form and include a contact phone number. Any requests submitted without this information will be denied.

3. A Deposit may be required for usage of the building – for weddings, large groups, etc. Deposit requirement and amount will be determined by the Administrator at the time of the request.

4. All areas used must be cleaned and restored to their original condition after the event is concluded.

5. All garbage must be bagged and taken out to the dumpster.

6. The building must be locked and alarmed before exiting.  THE PERSON DESIGNATED AS RESPONSIBLE FOR LOCKING MUST CHECK ALL EXIT DOORS AROUND THE ENTIRE BUILDING.  IF ANY DOORS ARE FOUND TO BE LEFT UNLOCKED OR OPEN AFTER THE EVENT, THE PERSON RESPONSIBLE WILL LOSE THEIR PRIVILEDGE OF USING THE BUILDING AGAIN. 

7. If you do not have a key or alarm code, it is your responsibility to contact someone who does, so they can let you in, and also close up after you.

8. Usage of the building will be approved based on church-related activities.

9. If chairs and tables are used and required from other rooms in the building, you are expected to return all furniture and other items back to their original setting.

10. If you change the seating arrangement in the auditorium, you are expected to return the chairs and other furniture back to their original positions.

11. If you require sound for your event, you will be responsible for contacting the sound coordinators to set this up. There may be a fee required for this service.

12. If you are using the kitchen, you are to take ALL unused materials with you upon exit. All foods or items left in the kitchen will be thrown out or donated to the church.

13. You are allowed to use items from the church kitchen as listed:
Punch bowls, trays, pots and pans, pitchers, candles, table cloths, etc. Disposable items such as napkins and plates should NOT be used – they are there for the children’s ministry. Do not use any food in the cabinets or refrigerator that you did not bring. Do not disturb any children’s ministry materials.

14. No rented items – such as chairs, tables, equipment, etc. will be brought into the building without the consent of the Administrator.

15. Any accident, building maintenance issues or problems must be reported to the Administrator within 24 hours of the incident.

16. No food is allowed in the main auditorium.

I understand that any infraction of these rules may mean suspension of the privilege of renting the building in the future. I understand that any damage to the building done by my guests will be my responsibility to rectify.
